Declaration of Major/Minor Form
Audit Change Request Form: Use this form to request changes if your existing MHS-eligible coursework needs to be rearranged to help satisfy your requirements in MHS or another program of study. Any changes will be approved by your MHS advisor and then sent on to the dean’s office and records office. Your MHS advisor can only request changes to the MHS portion of your audit. For audit changes for another major or minor, contact your advisor in that program.
Course Variance Request Form: Use this form to request MHS elective or concentration credit for a course that is not already on the list of eligible courses. Your request will be reviewed by the Director of Undergraduate Studies. If approved, it will be forwarded on to the dean’s office and records office. You can only request a variance once you have received a grade for the course. To receive pre-approval for MHS credit for study abroad courses, contact the Director of Undergraduate Studies at mhsdus@vanderbilt.edu.
