Please be advised that effective September 30, 2025, paper forms for doctoral academic actions will be fully phased out. All requests must be submitted electronically through the Doctoral Actions Workflow in Kuali.

This includes, but is not limited to:

1. Appointing an academic advisor
2. Forming or modifying a Ph.D. committee
3. Scheduling qualifying exams or dissertation defenses
4. Registering for independent study courses
