Academic Policies

Request a Letter of Good Standing

In order to comply with federal privacy laws, the A&S Registrar's office requires a signed release form for letters of good standing. To request a letter of good standing, please download a form. Forms also are available in the Academic Information Center located outside of 311 Kirkland Hall. In addition, you may fax a written request with signature to 615-343-8453. Please include the following information:

• Your Name and Commodore ID
• Name of Recipient (if known)
• Name of Institution
• Name of Office
• Address
• City, State, Zip
• Telephone Number
• Reason for Request (e.g., Summer Work Elsewhere)
